Subglottic Stent

Subglottic Stent

Code SG

The Subglottic Stent —also known as the SG stent or subglottic prosthesis— has, at its upper or proximal end, an 8 mm section in which the wall thickness is reduced: its standard 1.5 mm wall decreases progressively to zero. This creates, at that end, a low-resistance area intended to occupy the subglottic region, close to the vocal cords. The design facilitates deformation of the stent during laryngeal movements and glottic dynamics in phonation and swallowing.

Fixation of the stent in the trachea is handled by the remaining walls of the prosthesis, of standard shape and thickness. The subglottic stent may also be preferred for other locations, such as the mid or lower trachea or even the main bronchi, replacing a conventional stent.

When implanted in a main bronchus with its “subglottic” end in a proximal or cephalic direction, the transition from the bronchial mucosa to the interior of the stent is very smooth, without the step that accompanies the conventional stent, which could help reduce airflow turbulence and the impaction of secretions.

Clinical indications

The Subglottic Stent is especially indicated for lesions located in the subglottic region, and shares the indications of the tracheal and bronchial stents.

Available dimensions

The Subglottic Stent is offered in a wide range of sizes, with diameters from 10 to 16 mm and lengths from 30 to 70 mm. The SG code indicates the diameter and length (for example, SG12-40 corresponds to a 12 mm diameter and a 40 mm length).

Insertion technique

The procedure is performed under general anesthesia. The implant of this type of prosthesis can be carried out directly through the working channel of the tracheoscope or bronchoscope, or by using a conventional introducer for silicone prostheses. The airway is accessed with a rigid endoscope.

The length and diameter of the area to be covered with the stent must be properly established. A simple method to determine the length of the affected area is to mark the tracheoscope when its tip reaches the end of the lesion, and to repeat the marking after withdrawing it back to the beginning of the lesion. The diameter of the trachea or bronchus must be estimated by comparison with the known diameter of the endoscope used.

Retrograde implantation method
  1. Lubricate the mouthpiece of the introducer with lidocaine gel, avoiding contact between the lubricant and the operator’s fingers.
  2. Fold the Stening® along its axial axis and insert it into the prosthesis introducer through the mouthpiece.
  3. Remove the mouthpiece.
  4. Advance the tracheoscope tube beyond the lesioned area and place its distal end or bevel on healthy mucosa, exceeding the affected zone by about 5 to 7 mm.
  5. Place the introducer inside the tracheoscope.
  6. Press the ejector while withdrawing the tracheoscope by the same amount the ejector plunger advances: the plunger of the stent loader is pressed as the endoscope is withdrawn.

The prosthesis is thus released. If necessary, it can be repositioned with an alligator forceps, the maneuver being simpler if the stent is placed “below” the lesion.

Antegrade implantation method

Steps 1, 2 and 3 are repeated. Then the tracheoscope containing the introducer and the prosthesis is stopped 5 mm before the lesion to be treated, and the ejector plunger is pressed slowly. In this way, the prosthesis will be expelled toward the affected trachea.

Some stent loader models are not introduced inside the tracheoscope, but simply attached to it at its proximal end, from where the stent is propelled. For this purpose, the endoscope will have been stopped proximally or distally to the lesion as explained above, in order to push the prosthesis with the plunger provided with the endoscopic instrument. The stent will then travel through the entire interior of the tracheoscope until reaching the trachea. At this point, a sudden reduction in resistance to the pressure exerted on the plunger will be perceived, indicating that the stent has begun to exit the interior of the endoscope.

Correction of the stent position

The stent may require additional maneuvers in order to correct or adjust its final position. It is preferable to correct a stent that has been installed beyond the desired position than the opposite, since it is highly inconvenient to advance a prosthesis that has been released “before” the affected area.

To move a stent in a proximal direction, it can be grasped by its edge and pulled gently. We strongly recommend, for its precision, a maneuver consisting of grasping the stent by its edge as mentioned, and then advancing with the direct-vision optics inside the stent until visualizing its final end. Then pull the forceps and you will see the stent ascend through the airway. Stop the traction when you consider the position to be optimal.

Removal technique

Intubation is performed with a tracheoscope or rigid bronchoscope as appropriate. Easy to remove, the silicone stent must be grasped by its edge with an alligator forceps with sufficient firmness. The forceps is rotated about 360° so that the stent folds into an omega shape and thus loses its radial resistance to compression. Then the forceps is pulled, extracting the prosthesis together with the tracheoscope.

The proximal end of the stent may be introduced inside the tracheoscope. With this maneuver, the vocal cords are protected during extraction. Other methods of implantation and removal are also possible depending on the operator’s experience and preferences.

Considerations for the subglottic location

Naturally, when introducing the stent into the loader or into the tracheoscope or bronchoscope as the case may be, care must be taken to verify that its thin-walled or “subglottic” end is in the proximal or cephalic position, that is, “facing the bronchoscopist.” This is the only way for it to perform the functions that make it different once released within the airway.

The length and diameter of the stenosis must be properly determined in order to select a stent of appropriate dimensions. The final distance between the vocal cords and the stent must be equal to or greater than 2 mm. Bear in mind that distances within the affected trachea are difficult to estimate, since they will change when the patient assumes the standing position.

The recovery of muscle tone after the metabolization of the relaxants used during the anesthetic act adds a further difficulty in determining distances within the airway. Remember that you will additionally face the length changes that can occur in the trachea after it has been subjected to elongation and traction by the dilation maneuvers with the rigid instruments employed.

Although some or all of these circumstances may be present in all tracheobronchial recanalization and implantation procedures, they take on crucial importance in lesions of subglottic location, owing to the precision required for the stent to lie a few millimeters from the vocal cords. It will demand the bronchoscopist’s utmost skill and knowledge.

Removal

Intubation is performed with a tracheoscope or a rigid bronchoscope. Easy to remove, the stent must be grasped by its edge with an alligator forceps with sufficient firmness. Depending on the time it has remained in the airway, the edge of the stent may be confused with the bronchial wall.

After grasping the stent, rotate the forceps about 360°. The stent will thus fold, reducing its diameter and losing its radial resistance to compression. Then pull the forceps, extracting the prosthesis together with the endoscope. If preferred, the end of the stent may be introduced inside the bronchoscope; with this maneuver the vocal cords are protected during extraction.

Post-implantation care

Recommendations for follow-up of patients with a subglottic stent.

  • When an increase in secretions is noted, perform frequent nebulizations with warm isotonic saline solution.
  • Treat dental caries and maintain dedicated oral hygiene.
  • Endoscopic monitoring according to the frequency indicated by the physician.
