Stening® Class LSD

The Stening® Class LSD —also known as a conical bronchial stent or stent with lateral hole— is a conical Class stent that features a lateral hole to allow ventilation of the right upper lobe, when necessary.

It has all the special, well-known features of the Class stent, plus its lateral window, which gives it a special indication: preserving airflow in the upper lobe of the right lung.

Clinical indications

The Stening® Class LSD is indicated in conditions of the right bronchial hemi-tree that require keeping the airway open while preserving ventilation of the right upper lobe.

  • Bronchial neoplasms
  • Neoplasms in the right main or intermediate bronchus that do not affect the upper lobe
  • Neoplasms invading the tracheal carina or its slopes
  • Imminent atelectasis
  • Following laser photoresection, cryotherapy or electrocautery, to maintain airway patency
  • Bronchial stenosis
  • Post-infectious stenosis (tuberculosis, histoplasmosis with mediastinal fibrosis, herpes virus, diphtheria)
  • Post-traumatic stenosis
  • Bronchial rupture
  • Bronchomalacia
  • Bronchial amyloidosis
  • Following endoscopic resection of bronchial metastases

Available dimensions

The Stening® Class LSD is made in diameters of 12 and 13 mm. Single length: 40 mm. When the length is unnecessary, the stent can be shortened by cutting its distal portion at will, that is, its narrowest end.

Introduction technique

The maneuver does not differ from everything required and repeatedly mentioned here for the implantation of a bronchial stent. In the case of the conical Class stent with a lateral window, its placement is unique: the right main bronchus.

Since the purpose of using this special stent model is to maintain airflow from the main bronchus to the intermediate one without sacrificing the right upper lobe, Stening SRL prudently and insistently reminds you to ensure that the stent window is positioned inside the prosthesis loader or the bronchoscope in the proper orientation, so that, once released, it faces the operculum of the upper lobe—the final destination for which it was designed. This may require, in all LSD models, a “rotation” adjustment of the stent after release, in order to achieve maximum effectiveness and efficacy.

Removal technique

Intubation is performed with a rigid bronchoscope. The conical Class LSD stent is easy to remove, precisely because it is conical. It can be grasped with forceps by its proximal end and extracted.

Post-implantation care

Recommendations for the follow-up of patients with a bronchial stent.

  • Maintain the moisture of secretions when present, performing frequent nebulizations with warm isotonic saline solution.
  • Periodic check-up according to medical criteria.
  • Treat dental cavities and maintain effective oral hygiene.

Usage warning

The Stening® Class LSD is an anatomical stent, slightly conical in shape. Therefore:

  1. Remember that the stent must always be introduced inside the bronchoscope with the larger-diameter end in the proximal position, and the narrowest end in the distal position.
  2. If it is necessary to shorten the stent, it is preferable to make the cut close to its distal end, that is, near the smaller-diameter end.
  3. As its name indicates, the Class LSD models are only useful in the right hemi-tree. When loading the stent into the introducer or the bronchoscope, remember to orient the lateral hole of the stent toward the entrance of the right upper lobar bronchus, which is usually located at 3 o’clock. Verify this position beforehand during the bronchoscopic examination and once more after implantation.
  4. The device must not be reused.
